Training and Capacity Building

The Training and Capacity Building Department offers in-person and virtual trainings, technical assistance, curriculum development, referral pathways, and other services that strengthen organizations’ and communities’ ability to address existing and emerging public health concerns.

Refer to a Program

Physicians, care teams and health care organizations play an essential role in the prevention and management of chronic health conditions.

Often times, individuals with chronic illnesses may be asked to monitor symptoms of their condition, adhere to medication regimens, stop smoking, reduce alcohol consumption, modify diet, lose weight and increase exercise, and more. Patients often have difficulty performing these tasks, and this is where lifestyle change programs can provide the support and training needed to become healthier. (source: https://www.ahrq.gov/)

What providers and care teams can do:

  • Screen your patients for prediabetes to see if they can reduce their risk for type 2 diabetes by joining a lifestyle change program.
  • Browse the CDC resource library for flyers and informational brochures to share with your patients.
  • Refer your patients by using our Health Referral Network.
  • Contact our team to schedule a presentation on our program offerings, how to refer, and the benefits of lifestyle change programs.

HOW TO REFER YOUR PATIENTS

Step 1 - Screen patients for type 2 diabetes risk factors and test to determine clinical diagnosis. Here are the guidelines from the CDC.

Step 2 - Talk with your patient. It is important that their healthcare provider tells them their risk or diagnosis. Tell them that you are referring them to a DPP and it is important for them to attend.

Step 3 - Complete a referral form - here is a sample of the information a DPP provider needs for enrollment.

Step 4 - Send the referral to a DPP provider or the DPP Network Referral Hub - see below for further guidance depending upon the region of SEPA.

HPC will contact patients and register them for a virtual DPP series (while COVID-19 precautions are in place, all programs are delivered virtually, telephonically, or a hybrid).

HPC communicates registration and referral to the National DPP Provider vendor.

HPC will communicate referral disposition to the referring healthcare provider.
