Our Approach

We fulfill our mission through a unique combination of work that targets individuals and families, as well as the environment in which we live, work, and learn. Our goals are to reduce risk for injury and prevent disease, to help those living with chronic conditions successfully manage their health and to promote wellness.

Direct Service

Trained health educators, community health workers, case managers, and patient navigators work directly with individuals and families in their own communities.

Capacity Building

In-person and virtual training, technical assistance, curriculum development, academic-community partnerships, and other services can help to strengthen organizations.

Policy and Systems Change

Support with development and implementation of policies and practices can enable healthy behaviors.

The Food and Nutrition Service (FNS) Farm to School program aims to address inequities in access to therapy garden activities and improve access to local foods for children with intellectual and developmental disabilities (IDD). HPC implements the Garden of Eatin’ Project for special education students ages 5 to 21 attending Fairwold Academy. The Garden of Eatin’ Project expands the existing garden infrastructure to create an edible teaching garden, integrating agriculture and nutrition education The program also provides Harvest Tasting Events and Take-Home Harvest Days to improve fresh produce consumption and agricultural/nutritional knowledge among Fairwold students.

  

Health Promotion Council’s Nutrition and Active Living (NAL) holds lively and engaging quarterly events at PHMC’s Fairwold Academy with the USDA’s Farm To School program, “Garden of Eatin’.  NAL’s Paul McElwee, Connor Lightcap and Diana Molina led a fun and educational opportunity with Fairwold students, celebrating a “Green Theme” on March 17th, combining St. Patrick’s Day and a lesson in basil (how it grows in the garden, how it tastes and smells, and how to create a delicious recipe, like pesto! Yum!). 

With Paul as “Young McDonald – Old McDonald’s son”, Chef Connor in his chef coat (the kids asked if he was a chef on TV), and Diana’s bubbly and informative nutrition presentation, the Fairwold students had a great time learning, tasting, smelling and touching!

 

 

School-Age Direct Nutrition Education (Eat Right Philly) 

Health Promotion Council is a partner in the School District of Philadelphia's Eat Right Philly initiative that educates Philadelphia students about the importance of healthy eating and physical activity, and works with schools to create heathier environments. Health Promotion Council uses a variety of strategies to reach program goals, including classroom lessons, garden-based nutrition education, and after-school cooking clubs. Educators focus on topics such as choosing fresh fruit and vegetables, identifying food sources, and choosing water to stay hydrated.

Adult Direct Nutrition Education

Health Promotion Council provides direct in-person and/or virtual nutrition education in a variety of adult settings: health centers, grocery stories, senior centers, shelters, and pantries. Nutrition education focuses on improving the quality of life of populations by promoting and supporting healthy food choices and physical activity through evidenced-based nutrition education, cooking demonstration/food tastings, and interactive activities.

The Nutrition and Active Living Department provides training and technical assistance services for Philadelphia-area family shelters in accordance with the federal Child and Adult Care Food Program guidance.

These services include: menu development according to federal and City of Philadelphia Nutrition Standards, biannual culinary and nutrition trainings for food service staff, individualized training and technical assistance for food service staff, and ServSafe® certification training. Additionally, Health Promotion Council serves as the administrative entity to issue all monetary reimbursement and incentives to designated shelter providers that are compliant with the program in accordance with the Child and Adult Care Food Program contract.
