HPC's chronic disease management programs emphasize,
but are not limited to, the needs of low income minority groups. Direct
service programs are provided in the Philadelphia metropolitan area.
Diabetes and High Blood Pressure
HPC's major disease management program since 1991 is Taking
Control, a high blood pressure and diabetes project primarily
serving African Americans and Latinos. (The Latino diabetes project
is called Controlando Nuestra Diabetes.) Taking Control is
community based and embraces the self-empowerment philosophy. The
program is offered independently as well as in conjunction with primary
care providers to more than 400 persons a year.
Asthma
HPC has a growing involvement in city-wide asthma management efforts.
Currently, HPC is the lead agency in a project funded by the Robert
Wood Johnson Foundation and other sources involving the efforts of
a 40 member local coalition, Philadelphia Allies
Against Asthma.
